How to schedule vaccines:
1) Select month (month may be sold out – click the blue circle/X button in top right corner of the form to go back for more options).
2) Select date/time from the available options in that month (click again to un-select to choose another date). Disregard any drop-downs that may appear when you select.
3) Scroll to bottom of that month to “schedule appointment.”
DO NOT click “Schedule appointment” until you have selected a date/time. Use the blue arrow at bottom of form to go back for more options.
4) Complete the information form and submit. You should receive an email confirmation of your appointment.
Please be punctual. We operate on a tight schedule. If you are more than 5 minutes late, you may need to reschedule for an extra $15 re-scheduling fee.
